The form that you must have signed before you can fly solo in a Gyro. It is a self declaration of health and must be countersigned by your doctor. You do NOT require a medical in most circumstances.

Cross Country Signoff Sheet
Before you can leave the airfield with the intention of landing at another airfield you require a signoff from your instructor. This is the form.
